§ 3.05.040. Application for license.

(a)

Any person desiring to secure a license shall make application in duplicate to the city treasurer's office. The application shall be verified and accompanied by the license fee. Both copies of the application shall be filed with the city treasurer's office.

(b)

The application for a license shall be on a form provided by the city treasurer's office. A partnership application, and the application of any officer or director of a corporation and any stockholders holding more than five percent of the stock of a corporate application shall furnish the following information under oath:

(1)

Name and address, including all aliases;

(2)

The name of the owner of the premises upon which the adult entertainment establishment is to be located;

(3)

The address of the adult entertainment establishment to be operated by the applicant;

(4)

A statement by the applicant that he or she is familiar with the provisions of this chapter and is complying with them.
