§ 3.66.050. Same—Application; form; information to be shown.

The application for the license required by Section 3.66.020, shall be verified, upon forms to be furnished by the license collector, which forms shall contain the following:

(1)

The name of the person applying for such license;

(2)

The residence of the applicant;

(3)

A statement that the applicant is a citizen of the United States and not less than twenty-one years of age;

(4)

A brief statement of the business to be conducted under the license; and

(5)

The address from which this business is to be conducted.
