§ 3.84.210. Taxicab driver's license application—Information to be given.

  • (a)

    A taxicab driver's license shall be applied for in writing on such forms as the Treasurer may provide and contain such information as may be required, including the following:

    (1)

    The name, residential address, telephone number and date of birth of the applicant.

    (2)

    The type(s) of vehicle(s) which the applicant will drive under the license.

    (3)

    Whether the applicant has been licensed as a taxicab driver and if so, when, and by what municipality or state, and whether such license has been revoked or suspended, and the date thereof;

    (4)

    The number of times, dates and places, within the last five years, the applicant has been arrested or convicted for traffic violations, driving under the influence of alcohol or drugs and/or leaving the scene of an accident;

    (5)

    Whether the applicant has been convicted of a felony within the last five years;

    (6)

    Whether the applicant is now or has ever been registered as a sex offender with any federal, state, county or local government;

    (7)

    The applicant shall furnish a copy of his/her valid Kansas Class C driver's license.

    (8)

    The City Treasurer shall photograph the applicant. Such photograph will be attached to the applicant's license. Photographs are to be re-taken no less than every six years.

    (9)

    The applicant shall submit proof of attendance of a customer service class approved by the City Treasurer or enrollment in a customer service class. Such class must be attended within twenty-four months prior to the date of the application, or within ninety (90) days following the application date.
