§ 3.88.190. Same—Contents of application.

The license required under the provisions of Section 3.88.090 shall be applied for in writing on such forms as the license collector of the city may provide and contain such information as may be required, including the following:

(1)

Whether or not the applicant has heretofore been licensed as a taxicab driver or as a driver of such vehicles as those licensed under this chapter and if so when and by what municipality or state, and whether such license has been revoked or suspended, and the date thereof;

(2)

The number of times and places arrested or convicted for traffic violations;

(3)

Whether the applicant has ever been convicted of a felony or misdemeanor, giving particulars of each conviction;

(4)

The applicant shall furnish four recent photographs of himself not less than two and one-half inches by three inches, and not more than two and one-half inches by three and one-fourth inches, and submit to fingerprints by the police department. One of the photographs shall be attached to a copy of the application filed with the police department. One shall be attached to the application filed with the license collector;

(5)

The applicant shall furnish the names of three reputable persons residing in the city as references.
