§ 7.22.040. Issuance of permit.

Any person desiring to secure a permit under the provisions of this chapter shall make application for a permit on forms provided by the regulatory authority. Such application shall contain:
(a)

The name and address of the applicant;

(b)

The location for which a permit is desired; and

(c)

A description of the type of food establishment to be operated at that location.

Prior to approval of an application under this chapter, the regulatory authority shall inspect the premises of the proposed food establishment to determine compliance with all provisions of this chapter. No permit under this chapter will be issued unless the premises complies with all the standards for operation herein.
